The Google Forms logo, for Google’s online forms and surveys service, is designed as a purple document icon with rounded corners and a folded top-right corner highlighted with a darker shade. In the center of the mark there are three white circular dots stacked vertically on the left and three white horizontal lines on the right, like a list of questions. The current icon design belongs to the unified Google Workspace icon style introduced by Google in fall 2020.
Google Forms is a web service for creating forms, questionnaires, surveys, and quizzes, included in the Google Workspace toolset and Google Docs Editors. It lets you build different question types, share access via a link, collect responses and view a results summary, and automatically save responses to a Google Sheets spreadsheet for further processing. The service launched in 2008 and is widely used in education and business tasks, from event registration and feedback collection to internal surveys and testing.
